What this covers

This policy covers information collected through our website, the booking flow, the contact form and the Fennel app. Your medical records and the health information we hold as your care provider are covered by our Notice of privacy practices, which explains your rights under federal law.

What we collect

We collect only what we need to book and run your care:

• Details you give us: your name, date of birth, email, phone number and insurance information.
• What you choose when you book: the visit, clinician, clinic and time.
• Messages you send through the contact form.
• Basic technical information, such as your browser type and the pages you visit, to keep the site working and secure.

How we use it

We use your information to book and confirm visits, send reminders, bill you or your insurer, answer your messages and improve our services. We do not sell your information, and we do not use health information for advertising.

Who we share it with

We share information only with the people and services that help us provide care, and only as much as they need:

• Your insurer, to confirm coverage and send claims.
• Service providers who host our systems, send our emails and process payments, under contracts that protect your information.
• Others when the law requires it.

Cookies

We use a small number of cookies to keep you signed in and to understand, in aggregate, how the site is used. You can turn cookies off in your browser; some parts of the site may not work without them.

How long we keep it

We keep account and booking information for as long as you are a patient and for the period the law requires afterwards. Contact form messages are deleted after two years.

Your choices

You can ask to see, correct or delete the information we hold about you through this website, and you can unsubscribe from non-essential emails at any time. Write to us and we’ll respond within 30 days.
